CASE STUDY · SMILE DESIGN AND CROWNS

Four crowns to fix a reverse smile line, gaps and staining

A man felt his front teeth looked short and out of place. Grinding had worn them into a reverse smile line, there were gaps between them, and tea and coffee had stained them grey and yellow.

After treatment: four crowns following the line of the lower lip, with the gaps closed
Before treatment: short, worn front teeth curving upwards, with gaps and grey-yellow staining

01

The patient’s concern

He didn’t like his smile. His front two teeth looked very short and not in the right position, he didn’t like the gaps between his teeth, and the colour had become grey and yellow.

02

What we found

He had what we call a reverse smile line. Normally the edges of the top teeth follow the curve of the lower lip. His curved upwards instead, because grinding had worn the front teeth into an abnormal shape.

His front two teeth also sat forward and to the left, which opened up gaps on the right. Tea and coffee had added a lot of staining.

03

Our approach

We designed the new smile digitally first and showed it to him before touching his teeth, so he could ask for any changes. This step is how we make sure the result is the one the patient wants, and it is approved before treatment starts.

Crowns on the four front teeth could then lengthen the worn teeth, close the gaps and correct the colour in one short course of treatment.

SMILE DESIGN FIRST

Before any tooth was changed, we designed the new smile and showed it to the patient. They could ask for changes, and if they didn’t like it, they didn’t need to go ahead. The design also shows us how little of each tooth we need to adjust.

04

The treatment plan

1

Digital smile design

The four front teeth were prepared for crowns.

3

Four crowns

The crowns lengthened the front teeth, closed the gaps and improved the colour and shape.

05

The outcome

His smile now follows the line of his lower lip, the gaps are closed, and the colour is consistent across the front teeth.

06

Looking after the result

Protecting the crowns at night is usually recommended where grinding caused the wear

Regular check-ups and cleans

Cutting back on staining drinks, or rinsing with water after them, to keep natural teeth bright

07

Cost and payment options

Fees depend on how many teeth are treated, the materials used and any work needed first. After your consultation you get a written treatment plan with costs, before anything starts, and nothing goes ahead until you are comfortable with the plan. Payment plans are available for cosmetic treatment.

COMMON QUESTIONS

Questions about this treatment

What is a reverse smile line?+

When you smile, the edges of your top teeth should follow the curve of your lower lip. In a reverse smile line they curve the other way, often because the front teeth have worn down from grinding. It can make a smile look older or flatter.

Can crowns close gaps between front teeth?+

Yes, when the teeth also need changes to shape or colour. If they don’t, orthodontics or bonding may be a smaller option, and we would discuss that first.

What is digital smile design?+

Planning your new smile on screen from photos and scans, so you can see the proposed result before any treatment, ask for changes and approve it.

Can grinding damage front teeth?+

Yes. Over the years it can shorten the front teeth and flatten the smile. If you grind, protecting your teeth and any new crowns at night is important.
