What Is All-on-4 Dental Implant Treatment?

All-on-4 is a full-arch tooth replacement solution that uses four strategically placed dental implants to support a fixed bridge for the upper or lower jaw. Unlike removable dentures, the bridge stays securely in place, so there is no need for adhesives or taking it out at night.

The two front implants are placed vertically, while the rear implants are angled into stronger areas of the jawbone. This approach helps avoid the sinuses or nerve canal, meaning many patients can have treatment without the need for bone grafting or sinus lifts. Using four implants instead of replacing every tooth individually reduces surgery, treatment time and overall cost while providing a strong, stable foundation.

Your new smile is custom-made to suit your facial features, bite and preferred tooth shade. The bridge also replaces lost gum tissue to restore natural support for your lips and cheeks. Depending on your treatment needs, different restorative materials may be recommended, with all options explained during your consultation.

What All-on-4 Treatment at Hayat Dental Includes

Our experienced dentist in Sydney provides personalised All-on-4 dental implant treatment to restore a full arch of missing teeth with fixed, natural-looking results. Every treatment plan is tailored to your oral health, lifestyle and goals, using advanced technology for precise planning and long-term success.

Comprehensive assessment and digital planning

Consultation

Before treatment begins, we take a full medical and dental history, examine your teeth and gums, and take a CBCT scan. This three-dimensional image shows the exact volume, density and shape of your jawbone. Dental implants Sydney treatment is then planned digitally for precise implant placement.

Surgical placement and provisional bridge

X-Rays

Any teeth that need to be removed are extracted, the jaw is prepared, and the four implants are placed in their planned positions. A provisional bridge is fitted while the implants heal. This bridge is functional but lighter than the final one, and its timing depends on how stable the implants are once they have been placed.

Final bridge and ongoing reviews

Reviews

Once the implants have integrated with the bone, usually after three to six months, impressions or digital scans are taken for the definitive bridge. This is the bridge that you keep. From that point onwards, we schedule regular maintenance reviews to check the implants, the fit of the bridge and the health of the surrounding gums.

Not Sure Whether All-on-4 Is the Right Option for You?

The only way to know is through a proper assessment. A consultation at our Broadway practice includes a clinical examination, a three-dimensional scan and an honest conversation about whether full-arch implants suit your situation, or whether something else would serve you better.

All-on-4 Compared With Other Tooth Replacement Options

There is no single solution that suits everyone. What is right for you depends on your bone volume, your general health, your budget and what you want from your teeth day to day. Here is an honest comparison.

All-on-4 fixed bridge

A fixed bridge supported by four implants. It stays in place permanently, it transmits biting force through the jawbone rather than pressing down on the gums, and it does not cover the palate. It requires surgery, a healing period and a higher upfront investment than the removable options. Long-term success depends on good oral hygiene and regular professional maintenance.

Implant-Supported Dentures
ChatGPT Image Feb 19 2026 02 40 03 PM

Conventional full dentures

Removable, non-surgical and the least expensive option. Dentures rest on the gums and rely on suction, muscle control and sometimes adhesive to stay in place. A great many people manage very well with them. Others find that the lower denture in particular moves during eating and speaking, that certain foods become difficult to manage, and that an upper denture covering the palate reduces the enjoyment of food. Because dentures do not stimulate the jawbone, the bone underneath continues to resorb over time, which means that dentures need to be relined or remade periodically.

Individual implants and shorter bridges

Where you still have healthy natural teeth that are worth keeping, replacing only the missing ones with single implants or shorter implant bridges is often the more conservative choice. All-on-4 is a full-arch solution, and it is not the right answer if you have a mostly intact arch with only a few gaps in it.

Signs That All-on-4 May Be Worth Considering

All-on-4 is generally intended for people who have lost, or are about to lose, most or all of the teeth in an arch. You may be a candidate if any of the following apply to you.

Your full denture does not feel secure

This applies particularly to a lower denture, which has far less surface area to grip.

Most of the teeth in one arch are failing

Advanced gum disease, extensive decay or old failing bridgework across an arch often means that repairing teeth individually is no longer viable.

You have been told that you need multiple extractions

If a full clearance is being discussed, it is worth understanding your replacement options before the extractions take place rather than afterwards.

You have previously been ruled out for implants because of bone loss

The angled implant design means that All-on-4 can sometimes be completed in cases where conventional implants would have required grafting. It is worth being reassessed.

You have started avoiding certain foods

If you have quietly stopped eating things you used to enjoy because chewing has become difficult or unpredictable, that is a functional problem worth addressing.

You feel self-conscious about your teeth or your denture

How you feel about smiling and speaking in front of other people is a legitimate clinical consideration, not a vanity one

Comprehensive Assessment

Your All-on-4 Treatment Journey at Hayat Dental

We take your medical and dental history, examine your mouth and discuss what you want from treatment. A CBCT scan is taken so that we can assess bone volume, density and the position of the nerves and sinuses in three dimensions. Photographs and digital scans record your starting point. If All-on-4 is not suitable for you, this is the point at which we say so and talk through the alternatives.
Your scan data is used to plan implant positions virtually, before any surgery takes place. We determine where each implant will sit, at what angle, and how the bridge will be designed around them. You then receive a written treatment plan setting out staged costs and a realistic timeline.
Depending on your case, this may involve treating active gum disease, stabilising general health factors in consultation with your doctor, or arranging sedation. We will also help you plan your arrangements after surgery, including transport home, time away from work and what to have ready in the fridge.
This is carried out under local anaesthetic, with happy gas or intravenous sedation available for suitable patients. Any remaining teeth in the arch are removed, the jaw is prepared, and the four implants are placed. For a single arch, this usually takes several hours.
A fixed provisional bridge is attached once the implants are sufficiently stable to support it. We check your bite, make any adjustments needed, and give you detailed written aftercare instructions before you leave.

Over the following three to six months, bone grows into direct contact with the implant surfaces, in a process known as osseointegration. You will attend review appointments during this period so that we can monitor healing, check your bite and adjust the provisional bridge if necessary.

Once integration has been confirmed, we take impressions or digital scans for your definitive bridge. You approve the shade and shape before it is finalised. The bridge is then fitted, your bite is refined, and we show you exactly how to clean around and underneath it.

Implants require professional maintenance in the same way that natural teeth do. Regular reviews allow us to check the health of the implants, clean the areas you cannot reach at home and monitor the fit of the bridge. This is not optional, because long-term success depends on it.

FAQS

Everything you need to know about All-on-4 Dental Implants

We always take care of your smile

Surgery for a single arch usually takes several hours in one appointment. The full process, through to the fitting of your definitive bridge, generally takes between three and six months, because the implants need time to integrate with the bone. Your individual timeline will depend on how you heal and on the complexity of your case.

The procedure is carried out under local anaesthetic, so you should not feel pain during surgery. Afterwards, discomfort and swelling are expected for several days and are usually managed with prescribed pain relief. Many patients describe the recovery as more manageable than they had anticipated, although individual experiences vary.

Dental implants are designed as a long-term solution, and with good hygiene and regular professional maintenance, many continue to function for decades. The bridge that sits on top of the implants is a separate component and may need repair or replacement over time as a result of wear. Longevity depends on oral hygiene, general health, smoking status, grinding habits and attendance at maintenance appointments.

Possibly. The angled placement of the rear implants is specifically designed to make use of the denser bone towards the front of the jaw and to avoid the areas where bone loss is typically greatest. Many patients who were told they needed grafting for conventional implants can be treated with All-on-4 without it. A CBCT scan is the only way to know for certain in your own case.

The bridge is made to a shade and shape that you choose, and it includes a gum-coloured section that supports your lip and cheeks. You approve the appearance before the final bridge is manufactured. The result varies between individuals according to facial structure, lip position and the amount of tissue that has been lost.

No. The bridge is fixed to the implants and is not removed by you. It is cleaned in place, in your mouth, using a brush, a water flosser and interdental brushes. Only your dentist removes it, and only when this is required for maintenance.

Soft foods only for approximately the first two weeks, followed by a gradual return to a wider diet as advised. While you are wearing the provisional bridge, you will need to continue avoiding hard and very chewy foods. Once your final bridge has been fitted, most patients return to a broadly normal diet, although it is sensible to avoid chewing ice and very hard nuts, and to avoid using your teeth to open packaging.

They are different solutions with different trade-offs. A fixed bridge on implants does not move, does not cover the palate and transmits biting force through the jawbone. Dentures are non-surgical, considerably less expensive, and work well for a great many people. Which option is more appropriate for you depends on your bone, your general health, your budget and your priorities, and that is precisely what the consultation is for.

Yes. Both arches can be treated, either during the same surgical appointment or in stages. Treating both arches roughly doubles the cost and extends the recovery period, so we will discuss the practical implications with you during planning.

Smoking significantly increases the risk of implant failure and impairs healing. We strongly recommend stopping, particularly during the healing period. If you do smoke, we will discuss the specific risks in your case with you before you decide whether to proceed.
