A man had a mix of old restorations on his front teeth. Some were dark, rough and chipping, they collected plaque and stain, and his front teeth looked too prominent.

His front two teeth had been restored about four or five years earlier, but the teeth beside them had work that was 10 to 15 years old. Those older restorations were dark and rough, and they collected plaque and stain.
He also felt his front teeth were too prominent, or ‘bucky’, and that the sizes of his front teeth didn’t match each other.

Once we assessed the old work, much of it was failing and chipping away. The shapes and colours were mismatched, so there was no consistency between his old work and the rest of his smile.

To get one consistent colour and shape, we planned to replace the old work on the front six teeth with crowns.
A smile design came first, to help him visualise the end result and make sure he was comfortable with it before we started.
SMILE DESIGN FIRST
Before any tooth was changed, we designed the new smile and showed it to the patient. They could ask for changes, and if they didn’t like it, they didn’t need to go ahead. The design also shows us how little of each tooth we need to adjust.

Smile design
He saw the planned result and approved it before treatment.

Old work removed
The old restorations were removed and the teeth were protected with temporary crowns.

Six final crowns
About two weeks later he returned and the six final crowns were fitted.

The six front teeth now match in colour and size, the rough, stained surfaces are gone, and the front teeth look less prominent within the smile.

Sometimes. Where the teeth already carry old work, crowns can be shaped to reduce how prominent they look. Where the teeth themselves are out of position, orthodontics may be the better first step. The smile design helps show which applies.
After the old work is removed and the teeth are prepared, you wear temporary crowns while the final crowns are made. They look like teeth, so you can eat and smile normally in between.
As older fillings and bondings age, their surfaces become rough, and rough surfaces hold plaque and stain. Smooth porcelain is much easier to keep clean.
